How do I get started with your services?

Simply start with a phone call to Compassionate Management Care, Inc. (CMC).  A phone consultation is at no cost.  We will try to answer all of your questions and help you decide if retaining the services of CMC will meet your needs.   If you decide you would like to meet with us, for further discussion before making a decision to retain our services, we can schedule an in-home, no-cost consultation.  We meet with you wherever you or your family member(s) reside (within Brevard County, Florida) and at a time that is convenient for all. Upon making a decision to retain services with CMC and providing CMC with a required retainer, we begin the process of completing a "Comprehensive Assessment".  


What do you look for in the assessment?

We assess for existing needs in all of the following areas: medical, nutritional, social, environmental, financial, legal, support system, and entitlements.


What is a Plan Of Care?
Information gathered from the assessment is then used to develop a "Plan of Care".  The plan lets you know what the case manager will do for your loved one. It includes recommendations for services offered by Compassionate Management Care, Inc. and links to community resources.  Resources that will ensure all needs for your loved will be adequately met. This "Plan of Care" is discussed with you.  You decide with which recommendations you agree.  You decide upon the services you want for us to put into place.  Compassionate Management Care will initiate the plan when all parties concerned agree about the type of services.   


Are services from CMC complete once the plan is initiated?
After implementing the plan of care, all services  (private duty agency, care in a nursing home, etc) are closely monitored by Compassionate Management Care, Inc.  We schedule monthly visits and provide you with written status reports.  In addition, we will conduct as needed conferences with professional disciplines and be available 24 hours a day 7 days a week to assist with emergencies.  We are in constant contact with you and your loved one. 


What are your fees?

Following the consultation, a contract and retainer fee are needed to begin case management services.  Thereafter, at the end of every month, CMC will mail invoices.  Payment for services rendered is due upon receipt of invoice.  For further information regarding our fees, please CONTACT US by phone, fax or e-mail.
